In aesthetic nose surgeries, one of the most important discussions among both surgeons and people who want to have rhinoplasty is whether the surgery will be open or closed. Both techniques have advantages and disadvantages. Regardless of which technique the surgeon uses, the goal is a comfortable nose surgery without scars, the nasal skin does not shine, the sensation is not lost, the nasal tip is mobile, the natural shape is preserved, it looks non-surgical, breathes and smells easily, and is comfortable after surgery.
Preservation technique; It is a technique in which the soft tissues of the nose and the roof of the nose are preserved. Unlike open surgery, in this method, it is the connective tissue system of the nose that keeps the tip of the nose standing. In this technique, since the operations are performed under the cartilage and bone membranes, the mimic muscles, vessels, nerves and skin on the nose are preserved untouched. Thus, trauma is minimised and the postoperative healing process is shortened. The risk of pain, swelling and bruising is minimised. Thanks to the blood vessels protected during the operation and careful surgery, the anatomical structures of the nose can be seen much better and millimetric shaping can be made.
I use both the open and closed technique when necessary. I prefer the open technique in patients with severe deformity of the tip of the nose, who have undergone severe trauma before, and who have severe deformation. I use the closed technique in my other patients. I think that the surgical results and surgical comfort of the closed preservation technique are superior to other techniques.

In the past years, nasal tip stiffness and numbness that may occur after rhinoplasty was an annoying problem. Due to the stiffness of the nasal tip, the nose remained fixed without participating in the facial mimic movements of the face, revealing a surgical image. In addition, pain and stinging sensation caused discomfort in flu cases, while cleaning the nose, blowing the nose, lying face down during sleep and kissing. To overcome these problems, it is necessary to protect the connective tissues that provide support for the nose.
Scientific studies show that these ligaments form the details of the tip of the nose in natural, non-surgical noses that look beautiful. In fact, it is this connective tissue system that functionally allows the nose to breathe and keep the airway open. The nasal muscles attach to the nasal cartilages with the help of these ligaments and help keep the airway open. In preservation rhinoplasty, the connective tissues that provide nasal tip support are preserved. Even in the early postoperative period, the nose can be softer and more flexible. Movements that we frequently do in daily life such as lying face down, kissing, cleaning the nose are no longer a problem.
What is done in the examination and interview before rhinoplasty?

Detailed nasal examination before surgery is extremely important. In the examination, I examine the bones, cartilages, nasal flesh and airway in the nose in detail. I evaluate the nasal skin, nasal arch, nasal tip cartilages, nostrils, nasal base. In some cases after the examination, computed tomography or sinus film may be necessary.
After the examination is over, I take photos from every angle in the photo studio. I shoot videos from different angles to understand facial mimic movements, nose movements. After the shootings are over, we evaluate the photos and videos together. Then we move on to simulation on the photograph. We need to reflect the findings we learnt in the examination to the simulation and be realistic.

Therefore, everything should be planned very well before, during and after the operation and everything should be done under optimal conditions. Drugs such as aspirin, painkillers, green tea, etc. that can dilute the blood before the operation. herbal teas should be stopped 10 days before. After deciding on the operation, you will be given a list of foods and drinks that you should stay away from for a 10-day period.

In closed surgery, we can easily see and shape everything we want to see. In this technique, no structure is shaped blindly without seeing it. All structures are directly seen and shaped millimetrically with meticulous surgery. This is already reflected in the results.
Of course, there is a technique that each surgeon has a better command of. ‘It will not be closed to you’ approach is not correct. This situation has nothing to do with how arched, curved or low the nose is. We can make all kinds of aesthetic or functional correction with closed technique. Even in the second or third operations, closed technique can be preferred if the tip of the nose is not very deformed.
